Your child/children’s registration is NOT complete until we receive a signed Release of Liability Form and Acknowledgement. You can download the documents below and select the one for your site/location.
Please send the completed form to your site’s Uprising Ministry Coordinator along with your full name and your child/children’s name who will be participating in Uprising this school year:
